Bleach Chapter 378: “Eyes of the victor”
Cover page is Ichigo.

Yammi tries to crush Rukia in his released state.
With the last bit of her strength, Rukia does ‘Pale Fire Crash’ (Bakudo 33)
It’s slightly effective on Yammi
and he drops her to the ground.
As she’s going to hit the ground Ichigo arrives.
Ichigo repels Yammi’s cero.
He explains to Rukia that Orihime is fixing Ishida’s hand.
Ichigo avoids Yammi’s punch with shunpo.
He tells Rukia that he’ll finish quickly and head to the real world. Then they fight.
Rukia says that Ichigo’s eyes aren’t those of somebody who has won a battle.
And that something happened up on top of the dome.
Ichigo attacks and becomes the creature that defeated Ulquiorra.
He uses Tensa Zangetsu
The end.

When Ichigo arrives he’s shown to be underweight.
There’s more text than usual.
The pictures are a bit small.
Where Rukia falls to the ground Ichigo is somehow there in a cloud of dust.
She says the pics are smaller, but the last picture of ichigo is a large 2 page spread
Yammi has captured Rukia.
Chad and Renji are already down on their knees.
Yammi and Rukia have an exchange and he’s about to pound her into the ground,
when Ichigo saves her and says he’s going to hurry up and finish this trivial battle.
Rukia says something like: “That guy’s rank…isn’t the same as the rank of the guy you defeated.”
The 3 go off and watch the fight from a distance. It doesn’t appear they will be involved.
Ichigo says he’s going to quickly defeat him and then go to the real world.
He dons his mask and the chapter ends.
You can only see a bit at the end, but it looks like Ichigo’s mask has a horn. Maybe the one from before?

I believe it says something about the chapter ending when Ichigo dons his Hollow mask.
And something like "the last part of the book is, I think, Ichigo's mask grows horns or something but in the front?", but I'm not sure about it all. I don't want to mislead, but "mask" and "horn(s)" are definitely in there.

Begins with Aizen, Gin, Shinji
Aizen: She's dead. Your hopes of getting Ichigo and Orihime to save her are useless. All the gargantas that were used to travel though Heuco Mundo have been closed.
Gin: I over-did it, didn't I?
Shinji looking at Gin: You barstard...

Shiji puts on his hollow mask and strikes Gin.
Gin blocks with his sword, and they both battle.
Shinji then blasts a cero at Gin at a distance which appears to have hit him.
Suddenly, Gin appears out of nowhere behind Shinji.
Shinji: Shit
Gin: Ikorose. Gin uses his Shikai's ability and his sword extends at a high speed towards Shinji from behind.
???: Chikasumi no Tate
A blood red mist shield appears which stopped Gin's sword form impaling Shinji.
Gin: hm?
It's Urahara that saved Shinji. Tessai is also with him.
Urahara: Saved you again Hirako.
Hirako: heh, is that all you can say?
Urahara looks at his former vice-captain.
Urahara: Tessai use Jikanteishi (time freezing kido) on Hiyori to stop her wounds, and Kūkanten'i (teleporting kido) to send her somewhere safe.

*They all suddenly feel the presence of an amazing reiatsu.*
Ichigo: What the hell is this?
They see the giant form of Yammy Rialgo, and a severely injured Renji (in his bankai form), Chad, and Rukia.

The first panel shows Ichigo leaping from the tower and entering again the dome of Las Noches.
Bleach 378 - It Was Me
A cold wind blows the dust upon the dome of the huge fortress of Hueco Mundo.
Ichigo turns to Orihime as she continues to look at the spot where Ulquiorra was standing some seconds ago.
"Inoue...Did you see him ?", he asks.
She remains silent.
"Me...the hollow... that did all this... Was it him, right ?"
Orihime turns her eyes at him. Her expression looks sad and worried. "Kurosaki-kun..", she whispers.
Ichigo understood. He swore to protect his friends but this time he was the one who hurt them. Physically, but most important, mentally. He is pathetic.
"Cut it off, Kurosaki", Ishida comments as he is barely breathing. His left hand is still missing. " It wasn't you. The one who did this wasn't Kurosaki Ichigo. So get over with it..."
Ichigo turns surprised to his wounded friend. He feels terribly knowing that it was his own Zangetsu who stabbed Ishida's torso.
"Despite that, Kurosaki, if it was you, you would have never been able to stab me...Quincies are better and stronger than Shinigami", Ishida adds with a grin in his face.
"Yeah...I guess you are right...", Ichigo replies turning his eyes away from his friend. "Inoue, can I leave Ishida to your care ? I have to find Aizen".
"But, Kurosaki-kun...you are wounded...you have to rest a --", Inoue is trying to say but Ichigo is already gone.
With this brief moment in his mind, Kurosaki finally steps into the sandy interior of Las Notches. There are huge holes cracks in the blue sky of the fortress. It's now obvious that the sky is fake and it seems ready to collapse. Suddenly, his thoughts are being interrupted by a short earthquake. Not much away, it was only a matter of some inches that saved Chad from the huge hollow's punch.
"I am YAMMY RIALGO ! THE BRUTE of HIECO MUNDO. AND YOU ARE NOTHING BEYOND ME ! ! !", the giant yells as he lands a new, huge punch against Chad only to be saved by Zabimaru’s bankai form.
"Ho ? What is this ?", Yammy looks happily surprised.
Renji is barely standing. Despite having the thick body of Zabimaru between him and Yammy's punch, he really felt the hit. And from its looks Zabimaru will not be able to withstand such a hit again.
"Chad ! We have to keep moving", the vice-captain comments, "If he hits us, even once, it's all ov--shit ! "
Surprisingly quick for his size, Yammy rises his hand again and get's ready for another blow.
"Tsugi no mai, Hakuren !!!", a silver blast suddenly frozes the giant's right hand. "Chad, Renji ! Run !", Rukia yells as Yammy easily gets rid of all of the thin ice from his hand. The giant is completely unharmed but at least, Renji and Chad are now out of his range.
"You stupid chick. You thought that weak attack could hurt me ? Or do you think it’s safe if you are out of my hand’s radius? Let's see what you will do now !", Yammy comments as he prepares a huge cero blast from his mouth.
"Rukia !", Renji yells. "Get out of there ! ", but Yammy's cero is too wide for Rukia to dodge. "No !"
A huge blow echoes throughout the place.
"Hahahahaaha ! Nothing compares to my might ! " Yammy laughs but his words are interrupted as silhouette is standing at the spot where the cero was landed.
As the smoke and the dust clears the espada can see Ichigo standing in front of Rukia. He used his mask for an instant. It was broken from the overuse but at least it was enough to deflect that cero.
"Ichigo...", Rukia whispers.
